Membership Plans

Adult Plan $495/year Includes:

  • 2 Exams
  • All necessary X-rays (excluding CBCT)
  • 1 Emergency visit
  • 2 basic cleanings (excludes SRP “Deep cleanings”) OR 2 Periodontal maintenance
  • 2 Flouride treatments
  • 2 Oral cancer screening
  • 15% off all other services (excludes implants and Invisalign)
  • $500 off implants

Membership Details

  • Annual Fee: The membership fee is due in full at the time of enrollment and covers a one-year period starting on the enrollment date.
  • Non-Refundable and Non-Transferable: Once enrolled, the membership is non-refundable and cannot be transferred to another person.
  • Exclusive Benefits: Membership benefits can only be utilized for services provided at Hometown Smiles Dentistry.
  • No Combining with Other Plans: Our membership benefits cannot be combined with dental insurance, other dental savings plans, or promotions. Additionally, they cannot be applied to costs or services covered under workers' compensation, auto insurance, medical insurance, hospitalization, or any hospital charges.

Membership Advantages

  • Exclusive Discounts: Receive a 15% savings on most dental treatments. If you choose to use third-party financing, the savings will be adjusted to 5% due to merchant fees.
  • Comprehensive Coverage: Our plan covers a wide range of dental services, ensuring that you receive the care you need at a fraction of the cost.
  • No Pre-Existing Condition Exclusions: While treatments started prior to enrollment are not eligible for savings, all other dental services are covered without any exclusions for pre-existing conditions.
